Surging and bogging down

My car boggs down from 0 to 40 mph...all
the way to the floor at 40..if i take my foot of
gas and reapply it cathes and the car xooms
off...after driving over 45 mph its fine...now
when i slow down and the car gets down to
40 mph deceleration it surges and idles
high.  It will surge sitting still also and has
started dying out. I have changed tps,fuel
pressure regulator and maf sensor

When your car dies on you does it doe at stops? Does it start right back up?
