When i take of oil cap while truck is on the
engine settles down, unwinds almost as it's
about to go off but doesnt when i put it back
on it gets loud sgain

Check your PCV (positive crankcase ventilation) valve. Sounds like you're building up too much pressure in the crankcase and opening the oil filler cap is relieving it.

Definitely blocked PCV- those 5vz/ 3vz motors are known for PCY blockage- the best solution is to remove the PCV, drill it out, replace it, and install an oil blow-by-catch-can.
