What is this? And what does it do?

I have been having trouble with my truck not
running right. It would not idle without dying.
Unplugged the hose from the thing in picture. And
it  idled decent. Put my finger on it and it died. I
know it has something to do with the vacuum. Any
help would be greatly appreciated.

Looks like your PCV. A plugged PCV can cause a buildup of exhaust gases in crankcase and restrict air entering engine causing rough idle or worse. Pulling the hose off alleviates the gas buildup and lets engine breath better, try replacing it and cleaning the hose, as it may be choked with carbon.
