my minivan vibrates when accelerating 35-45 mph. what could it be?

Check the following: CV JOINTS, a deformed rotor, brake pads, wheel alignment, Tires could be out of round, Un-even tire wear,

My Toyota sienna LE, 2009, FWD, V6. Shaking at 40 mph ,already replaced 2 mounts , still vibrates at 40 , what could it be!!! Carlos
