The lever on the left is for selecting gears in the manual transmission. The smaller lever on the right is for your four wheel drive mode. It controls the output of the transfer case. The modes are four wheel high gear, rear wheel drive, four wheel low gear. The transfer case can also be put in neutral.

Only change the transfer case modes when stopped or going very slow.

This is what you are looking at http://www.quadratec.com/products/52253_100.htm You will not be able to get individual parts, only the full set.

All that is is a shifter kit, there is nothing special about your transfer case. You did not say what was wrong. It is called a split shifter. You can replace the whole shift kit pretty cheap or convert it back to a single shifter if you would like. I like a split shift because it give you Low 2 wheel drive.
