What is wrong with manual transmission if Engine revs high and goes slowly

What needs to be replaced exactly on my
manual transmission  2005 5 speed by.  
When I put in first or any gear it revs and
moves slowly at first and then once moving
if I gas too much it seems to stop giving
power

Try bleeding the clutch line and if that don't work, then the clutch is probably going out.
