Pulling to one side tire flew off

I got new tires had them rotated
and a couple days later the tire
flew off. The steering wheel is
slanted. Is that the result of
getting a alignment is needed?
Please help

Yes it was and the whole tire left the van. I had just got them mounted but i wanna know do a bad alignment have something to do with it also

Wow! That shop owes you an apology they didn't get somebody killed. This happens a lot and they know it. Get back to them to check and redo all the work they did, including damages and a wheel alignment. They are at fault...Not You.
