Fan to cool the engine emits hot air instead of of cold one, what is the cause and how can you correct it

the fans come on and it still over heats ? is the coolant topped off , what does it look like ? get a new thermostat or test the old one in a pan of hot water , pull the oil fill cap and look at the bottom what do you see, what does the exhaust look like ? Could be the radiator is clogged , are both hoses ,in and out , the same temp ?
