miss whenfirst started

I have a 3500 chevy single wheel drive. I noticed I had a miss when I first started it and when I drove it 5 or so miles it seemed to go away. Now it is getting worse and when you don't start it for a week or so it is really hard to get started again. I had a tune up, when I first noticed it, and it didn't help, plugs cap rotor and wires. The shop just gave uo any help??

I've sprayed carb. cleaner around and no change in rpm .Also it idles ok once running. Seems to have a miss. This morning I put injector cleaner in it. I'll see what it does. Maybe check fuel pressure?
