Transmission gone ?

as i took off form the lights i felt i very small slip in the gear ever so small
then as i was driving low speed and i could feel almost like the hand brake
was getting pulled on slowly  (was sitting in traffic) as i went to take off i had
to use a little more of the peddle each time i stopped as i lifted the peddle the
car would come to a stop almost straight away i put it in reverse and it would
not move then i put the foot down and it took off with a leap its got a muffled
noise coming from under neath  witch is a little louder from the inside of the
car witch is happening wile the engine is running what could this be ?
