Have you checked your water pump? Check the seal around it. Also is there a check engine code? Sometimes the coolant temperature sensor can go bad . Based on leaks I would gather more towards the water pump. An inexpensive fix if you can do it yourself.

First make sure radiator cooling fans come on when they should. If the fan motor can run, next check fan fuse and fan relay. If leak is not external, you may have a failed head gasket. Smoky/steamy white exhaust? Not good.
If you have cap off radiator and van running you should not have bobbles or gushing water out of radiator if u do head or if lucky just gasket
