rev limiter

so this is a project of mine and when i got it it did not run. fast forward to
today and the truck is running idles good and then when i open the throttle it
revs up till i get to about quarter throttle and then it surges no matter how
much more the throttle is opened. once i come back to 1/4 throttle it smooths
back out i have watched the injectors (brand new) as the surging occurs and
they have a good spray right up untill the surging starts then they just
intermittently spray like its trying to lean out the mix when it should be adding
more fuel. i have pulled all codes and repaired/ replaced any and all sensors
fuel pump/filter all relays and the injectors as noted above im  reaching my
wits end and struggling to find the why to this issue any advice is appriceated

as a side note the engine sounds like it is hitting a rev limiter but why at such a low rpm ?

Have you installed a fuel pressure gauge on the rail to see what the pressure is at idle and when you rev it up
