I have a 1991 ford ranger and don't know if the fuel pump or fuel filter is going or maybe something else all together. When I start it, it idles very rough like it's trying to die but usually doesnt.

Do plugs, wires and a cap and rotor. Change the air filter and you should be good. Change the fuel filter if it hasn't been changed recently 50000 or so . Before you dig in to the pump. It's probably just not getting full spark
