The interior lights do not turn on when the driver's door is opened, but work when the other doors are opened

My guess.. The driver's door switch is faulty..

Any idea where the switch actually is ? I've taken this door apart many times, I didn't see it.

I thought I would post the fix for this in case anyone had this problem. First, the switch that activates the interior lights is at the back of the door lock assembly on each door. With the panel taken off, you will see a black and blue wire towards the back of the door panel toward the outer edge. I popped the holders off the panel and followed it to the back of the switch, there was a break in the wires. I re-attached and used shrink tubing to hold in place. The problem occurs because the door panel gets loose on the driver's door and the wire rubs against the edge inside, behind the panel. All is well and the lights work fine. Doesn't hurt to add a shot of WD 40 into the door locks, keeps things working as they should.
