could be a few thing, camshaft sensor, Throttle body sensor, the throttle body may need a good cleaning. When is the last time you did a complete tun-up in the truck
REPLACE THE PLUGS AND WIRES PUT A FUEL PUMP AND A FILTER
If the truck is not taking off in first gear it will be really slow off the line.
