what would cause the serpentine belt to slide into the motor? we've checked the pulleys and all are in line, what could cause this to happen?

the car came in with no belt, we replaced the tension pulley, checked all pulleys,

Was this a new belt that you just installed ? That is one thing about a serpentine belt you think you can remember how it went on but you don't so many combinations you may think you got it right but it is not. Most part stores like auto zone will give you a print out of how the belt is routed on the engine for free.
