Where can I find the parts to lift my 1991 Subaru Loyale stationwagon 4 inches and put 29 inch rims and tires on it?

I am trying to lift my subaru loyale wagon up 3 to 4 inches and I can't seem to find way to do that. I also want to find some websites where I can buy the lug nut adapter plates to switch it from four on four lugs to five lugs. Can anyone point me in the right directions? I want it to look similar to this:

All of the lifted Subies are custom built, I have yet to see a kit for any. Usually the do a body spacer lift and have the cv axels lengthened. On the lug adapter plates I am not sure, if you have trouble finding them you could always have a local machine shop make you some.
