Idle speed too high on startups and
varies while driving

The car should idle fairly high when cold. Maybe 1,500 rpm or so. What are the idle speeds you are seeing?

Cold starts at 2500 rpms and very slowly comes down to 1200 after 10 minutes. But I have to rev it sharply to settle at the 600rpm But then varies while driving ...rpms will rise sharply intermittently

Anybody out here have any advice, clues as too why my Avalon does what it's doing? Btw, vehicle is in mint condition with 125k miles

Maybe the throttle is sticky and needs lubricating. Your IAC (idle control) may be going bad. You should also check for vacuum leaks.
