Serpentine Belt keeps slipping off

My serpentine belt on my Highlander keeps
slipping off even though there is a good amount of
tension on it before I start up the vehicle. I’m very
limited in my knowledge of pulleys and such. Any
advice on what to check to make it stay on?

Also I have put the new belt on twice and every time it runs for about 10 sec. then it slips off again.
