How many miles do you have on it? Have you changed the timing belt?
Brother, those belts have to be changed every 120k- the 5vz, 3vz, 1mz are notorious for acting wonky when the timing belts get slack--- since your truck doesn't have coil on plug, it isn't likely you have issues with the ignition system. Start with the belt and also ensure the vacuum lines aren't fragged either.
