Poor acceleration especially uphills. What could it be ?

My van is having a issue accelerating and going up
hill. I've changed the fuel filter and nothing
different. What could it be ? It's my only vehicle and
I have 4 kids and need this fixed asap.

My 2003 venture is doing the same thing. It was showing codes for the Oxygen Sensor Bank 1 sensor 2 Bad, Insufficient Flow FromThe EGR Valve, and a Multiple Cylinder Misfire. I replaced both Oxygen Sensors, plugs, wires, and cleaned the EGR Valve. Still showing Multiple Cylinder Misfire and No data from the Oxygen sensor Bank 1 Sensor 2. I have decided to replace the Ignition Coils and replace the Oxygen Sensor again. Does anyone have any other ideas?
