2001. Chevy Venture leaks coolant from a hose on the back of the motor

Find which hose is leaking and either change the hose or tighten the hose clamps.
It looks like it doesn't have a clamp it has metal things that go around the hose

If you can access the hose clamps, then I would change those out to modern ones. AutoZone sells them in a variety of different sizes as well as a pack of different sizes for I believe around 5-10 bucks per pack.
It looks like a heater hose or the same way that the a/c hoses look like
