What causing water and black smoke coming out exhaust pipe on 1996 Gmc 350 vortex 1500 pickup

Small amounts of water is normal in the morning when your engine is cool. A lot of water/white smoke could be a coolant leak from a bad head gasket. Black smoke is from running rich.
