Why won't the transmission shift out of 4th gear when in manual mode

Automatic transmission works fine in Park, Reverse and Drive. But when I put it in manual mode it won't shift out of 4th gear.

Need some more info here. When you shift to manual mode from a stop does it automatically put you in fourth gear and won't change? Or can you shift all the way up to 4th gear but cannot go any higher?

No matter what state the car is in, the only gear I can get when I shift to manual mode is 4th and it will not shift to any other gear.
