I just had the same problem. Had to replace the EGR Cooler and the radiator. Cost was 4000.

Everyone with a powerstroke diesel truck should watch Bill Hewitt @ powerstrokehelp on youtube. Amazing stuff! Has video about radiator cap($5 part!?) spring is insufficient for holding presure(bad cap)and can lead to head gasket problems, etc...(has tons of useful info and solutions). Says to replace with cap #10238(Advance Auto). Old cap not holding pressure and coolant gets too hot before fan kicks on leading to issues. Also mentions after replacing cap, have to monitor system and check for leaks/problems because now cap is sealing. Really knows his stuff about powerstrokes...everything you want/need to know...including back history on FORD's motor debacle!!
