why does the rev counter jump when driving and car has a jerk when it does
this

Could be a number of things such as a transmission speed sensor issue, wheel sensor, throttle body, or even a module acting up. The only way to start a diagnosis is to have the pending codes read. Sometimes a regular obd scan will show something but for specifics the STAR diagnostic program will pinpoint stored codes. In the meantime you could check all your fluid levels especially the transmission. Good Luck!
