Don't touch bulb with your fingers, bare skin. Use some type of moisture proof gloves. Sometimes moisture from your fingers might cause a hot spot on the bulb? Just a thought.
Is the seal on the headlight assembly still good? Any sign of moisture in it? Next time you install a bulb, use a liberal amount of dielectric grease. They sell little tubes of them at any auto parts store.
