2006 Avalanche runs great, but heat only works when I turn dial to 4 or 5

If blower only works on high speeds, suspect the blower motor resistor

It's your blower motor resistor. You can purchase them on ebay for about $14.00 Also they are very easy to replace. Lower your glove compartment, (or access it from underneath) and you should see it hooked with two small screws. you probably need a 7 mm socket to remove them.
