This seems to be a manufacturer's design flaw, there are a number of complaints about the transmission disengaging from the engine under certain conditions. Check out NHTSA.gov for additional info on TSBs and recalls. They did recall a number of GM vehicles (including Equinoxes) for a driveshaft issue that could lead to a roll away. Hard to say what caused yours to roll. You have to follow the parking procedure in the manual closely, some of these new cars are temperamental about going into park.
