Any temperature increase in the coolant will generate pressure which is normal. What exactly is the issue?

Have done quite a few head gaskets in my lifetime and just kind of puzzles me when it's got almost instant pressure to the radiator. Usually you can let a car run and warm up with the radiator cap off but does bubbles back up and out almost instantly.
