I would look first at the CV joints and make sure they are not about to come apart. They can lock up in an awkward angle if severely worn and create a big out of balance problem. If you can rotate the tires from front to back, that might eliminate possible tire issues. Worn inner tie rod ball joints can cause a bad wobble at speed, but you should be able to tell that by jacking the wheel up and move the back of the wheel in and out with a hand on the tie rod connecting arm that goes to the gear (inwards) it should give you a "click" if it is worn out and this is serious if any suspension part fails. You can take it to a "Pep boys" or similar place for a free estimate and they may be able to narrow it down. Strut towers on driver's side are known to rust through on these vans. Open up the hood and look at the top of the strut tower and see if it is rusting through.
