I would say most likely a sensor, you can put a direct power source to test the fan. there is usually a temp sensor and temp switch one sends a temp to your dash and one clicks on the fan

The relays are on the Right side as Looking under hood. Beside upper Radiator Hose. But the Problem is with the Program. I replaced Cooling fan & Radiator with a Aftermarket Fan & bought a Predator Tune. Must Flush Cooling system completely. Fill with Dexcool to keep the Antifreeze from eating up Aluminum. The fill & expel all air in Cooling system. I had to remove the Intake heating hose & squeeze the upper hose to get all air out.
Also check that the Speedometer is functioning properly. Get a OBD-II Tester to check Trouble codes & that the wiring to Fan is not Shorted or Melted. Mine was. Had to run New wires & connector for Fan
