Pretty common on our platform. New fan clutch is in order, suggest buying one with a lifetime warranty if you do it yourself. After new one is installed and you hear ticking, get a different one. If you do it yourself, do yourself a favor, cut a notch in the shroud at the upper radiator hose, so the shroud can be pulled right up. Not doing this will cause hours of fighting and bending the shroud to remove and install it. Will make this job cake.
