Did the Z28 ever have 3 duces from the factory?

The GTO did and the big block Corvettes did, but not the Camero. This was in the 60s when muscle cars were kings of the road, and MPG was not considered. Also the Corvette side draft six cylinder engines had three carbs.

Also some Pontiac 400 s hade the tri set up

I owned a 1968 Z-28 with 3x2s stock from the dealer.
