Fan making noise

While in park and running fan makes a very loud
noise like the fan is hitting the fan guard. When the
car is off I can spin the fan by hand. Does it need
to be replaced or is it just missing a part or need to
be tightened?

Check the fan motor, it could have gone bad. When you move the fan by hand, can you see if it is hitting the fan guard? Does it feels tight or is it kind of loose? It is normal to be able to be able to spin the fan by hand.

the fan may have a clutch making the noise or a bolt may be loose , the clutch may have a left hand thread , look for any damage , take a look when it's running

The fan is powered by an electric motor. Yes, check the fan housing for loose bolts or parts.
