I have a 1992 GMC Sierra 4x4 and it will not start without putting gas or starting fluid in the throttle body.

You need to check the fuel pump, and the pump fuse. this is the first you need to do. if it is working check the fuel pressure too it should have at least 35psi.

I have good fuel pressure. I have been told that its the ECM. The truck runs perfect as soon as it starts. I just have to spray it to start it.

I am not specialized in american cars but for the year of this vehicle i would check if it has a cold start injector and check it. If it runs good i don't think it is the ECM but it is possible. The injector could be clogged or the fuel filter, maybe it needs maintenance, spark plugs for example. check to see if you have injector pulse and spark when you try to start it to see if it's the ECM. Maybe you have a low compression issue, check the compression. if you have more info i can help more...
