What could cause this to happen.

My sons 2002 camry v6 suddenly started a gear
grinding crunch from what sounded like the driver
side axle. Upon further inspection i realized its
coming from the transmission. I checked dipstick
and fluid, all though full, was dark and with tiny
metallic flakes.  Transmission has never slipped,
never made a sound, and although has not been
flushed since he bought the car a year and a half
ago, it has never given a single warning sign of any
issue. Its actually been a very good running car.  
Any reason known why the transmission would
suddenly do this and what kind of damage it could
be. ???
