I'd perform a compression test, something is causing some extreme pressure buildup in the crankcase, might have a bad piston ring, or hole in a piston. Should check PCV as well. You always have some blow-by gases in the crankcase, which usually get sucked back into the engine and burned via the PCV/breather system. Check the oil condition, a bad PCV may result in a milky looking residue around filler cap or in oil.
