07 PT cruiser overheating

I have a 07 pt cruiser the radiator was
replaced last summer, after about a hour of
driving the bar starts to rise and you hear
bubbling. Stop the car and it’s coolant
bubbling leaking out of the clear tank under
the the hood. When the car cools all the
coolant that was bubbling goes back. I was
thinking a head gasket? Or thermostat?

I also can’t use my a/c it blows out like warm air doesn’t really cool. I was told put Freon and it should be okay.

When replacing the head gasket do you need the whole set with the other little pieces or just the gasket and head bolts.
