You will have Fan#1 and fan #2 relay,the 1 is for radator fan,#2 is for AC when on,they are the same and cost runs about 13 to 18 dollars a piece.

I've read on the relays there are four prong or five prong. Which one should I get. Would you know there number

The relay would depend on the engine size,best way to know is remove one and take to parts store with you to make sure you get the right one.
