I had this exact problem in my 1995 ford f150. Pull out your blower motor, its very easy just unplug the wires going into it and if it has a hose pull it out and undo a few bolts. Look at your blower motor, if there are any holes or noticeable damage it needs replaced. If not try to spin it, it should spin easy and not be loose. A blower motor is cheap off Amazon, I do not recommend buying from a parts store cause they will likely charge more. Just make sure they look the same and same hook ups. That should fix the sound problem and make the heat work better. Now for the only working on high part it is most likely the blower motor resistor. Very easy fix just take ground off battery, undo clip on resistor, take off two bolts, and replace with new resistor. This should take care of your problems.

If the fan only works on high the problem is a bad blower motor resistor. If the fan is excessively noisy you should clean the fan blades and you may need to replace the blower motor.
