how do you check codes on a 1998 caravan?

i have a 1998 dodge grand caravan with a 3.3l v6 flex fuel engine. it runs rough at idle
and when trying to rev it. it won't rev past 2500 rpm either. i have tried to check codes
without a scanner using the "on-off" 3 times method as well as the "hold-the-trip-and-
reset" method. the check engine light is on but no codes come up except "code 999"
which i believe is normal. i have changed spark plugs and fuel filter. am i missing
something? maybe it's a bad pcm?

i was able to hook up a scan tool to my van and the only code that comes up is for a bad EGR valve. i can't see this causing my problem though. i was also able to get the engine to stay running for a bit, although it ran rough the whole time. the fuel pressure is at a constant 59-60 psi with the engine running. also, there was no fuel smell when i opened the throttle body with the engine shut off. i am leaning toward a bad PCM with a bad fuel injection circuit. can anyone help?
