JEGS has various parts that could help you fabricate a hose. You could also take measurements, go down the the auto parts store and look for a hose that is close. A hose that is close can be modified to fit.

Dayco (belts and hoses) has a lookup by dimensions at www.daycoproducts.com. They are one of the largest manufacters in the world and their parts are available at just about every auto parts store you could fathom. The catalog should give you at least a couple of choices (some trimming may be required) and a part number to use/cross reference to another manufactuers. Hope this helps
