My 95 Honda Civic is making a "stutter" sensation when I accelerate.

My 95 civic started jerking when I accelerate. At first I noticed it when I shifted to 4th gear only, now it occurs when i shift anytime above 2nd gear.  It not that violent like a stall out, but its definitly worrying me. Can anyone please suggest somthing I can look into, thanks

Does it make when ur engine cold /like started and in 10 mins? It may be cause of cool-cold exhaust.
