Why after highway driving, car jerks when shifting between 1st and 2nd gears (upshifts and downshifts)?

I have a 1998 Ford Windstar 3.8. After highway driving, car jerks when shifting between 1st and 2nd
gears (upshifts and downshifts). Problem continues even after having changed the transmission twice...
What else could it be?

Try turning the cruise control off before exiting the highway. If problem persists it can only be the torc converter.
