It is a "tee" fitting generically speaking and may not be available from Ford now. Is the fitting leaking or the hose?

It may also be called a heater hose fitting or tee. Which engine is this on?

A hardware store is where I would look for a replacement but all you may need to do is take it apart and use some good thread sealant to stop the leak.
