Is it idling a little rough? Try running an engine cleaner in the oil. The VVT will clog if it is not regularly serviced causing build up. This causes exactly what your describing. I would also look at the mass airflow. The lights were cleared when the O2 sensor was installed. They turned back on because the problem wasn't fixed. Toyota will be little to no help diagnosing this. I speak from experience. Find a good indecent mechanic that works off more than just codes.

I have a 2006 Rav 4 that had the same problem which occurred sporadically for a couple years. A friend suggested to fill it up with gas and re-tighten the gas cap but just one click as per instructions. That made the lights go off but they kept coming back on. I bought an off brand gas cap and that worked a couple weeks until I couldn't get the cap off at the pump. I bought a "Toyota" gas cap and problem solved -no more lights.

Our 2007 6-cylinder RAV4 with 80k miles had the VSC, 4wd, & Check Engine lights all come on, and the engine was in a rough idle. After a few disconnecting the battery and reconnecting, the lights appeared. Another Forum said it may be due to a faulty spark plug coil. I replaced all six coils and plugs. The car now idles perfectly, and the lights don't come on. There's also a great YouTube video for the 6-cyllinder RAV4 from "2CarPros".

The big picture to help find it, noticed this as I was cleaning my mass air sensor.

With 47,000 miles,my Rav 4 2011 V6's three lights were on, then off, then on again and it appeared related to the gas cap which seemed too easy to click more than one time. I replaced with an official Toyota gas cap and viola, fixed. I avoided running to a dealer and being given the run-around with replacing sensors. I recommend trying the easiest fixes first. the new gas cap cost $20.

My Rav4 2008 had 3 lights always on (185,000 miles). I followed and checked air filter connections and wires, and everything were fined. I found out that my air filter was very dirty and blackened. Replaced new air filter ($12) and cleared error codes. After 2 days, the warning lights didn't display yet. So far so good.
