Dodge won't idle

2001 Dodge Ram 1500 RWD idles and revs just fine in
park, but barely runs in reverse, drive, and even neutral, if
you rev it in neutral it gets really choppy and rough, dies
when you let off. Put it back in park and it runs just fine
again.
