whats wrong with my transmission?

I have a 2002 Chevy Suburban 1500 and the transmission is having issues to say the least. When ever it warms up it starts to make a belty sound then loses almost all power. the filter has not been changed in a long time. Does anyone know whats wrong with it?

looses power as in the engine cuts off, or loses power as in going up a hill and is not as fast looses power?

The best way to describe it is it is like a car going up a steep hill in 6th gear from a stop.
