reliability and what mileage to worry about mechanical issues, when to expect issues at what mileage

This is a 15 year old car. If you don't know how it was maintained and driven you cannot predict the future. Lexus cars can run a very long time when properly taken care of.

Usually around 100 to 150K is when you are replacing suspension and other parts that run off the serpentine belt, like the water pump and alternator.
